Hazard - United Water cover - Railway Tce Edwardstown/Plympton

To Marion Council council@marion.sa.gov.au and United water uw.adelaide@uwi.com.au
Attached is a photo and map of a service cover that is sunken by more than 30mm.
 
This is on a bikeway, near a corner, AND approx 1 metre from the kerb (in the path of a cyclist).
 
Impact will cause injury or damage.
 
Going around it will cause collision with passing vehicle not expecting the change of path of travel.
 
Please remove the hazard.

Man-made Hazard - Corner of King George and Whyte

Please take action to remove the hazard created by workers cutting a hole in the bitumen at the corner of King George and Whyte (see attached photos and map).
 
Please do not bicker on who is responsible. Just remove the hazard.

In the event of an injury, damage or related accident, the person who created this hazard will be the direct causation and liable.

I have contacted DTEI on 1800 018 313 (road and traffic light report hotline).

They claim it is the responsibility of United Water.

This is odd, as the cover clearly has STORMWATER embossed in the cover.

Hazard - Service Cover - Railway Tce Bikeway

Hello

Please fix the service cover on Railway Tce Ascot Park, corner of Third ave (map and photo attached).

The service cover is greater than 40mm below the road surface and has a 90 degree rough edge of bitumen.

A cyclist will be injured from impact, or damage will occur to bicycle.

A cyclist avoiding impact will be at risk of collision from a passing vehicle not expecting the cyclist to change line of travel.

I realise that there is often negotiation required to establish who is responsible to fix service cover hazards.

Please mininise the time it takes to remove the hazard. This will miminise the chance of injury or damage to a cyclist, and the subsequent claim for losses.

Feedback on progress to remove the hazard is appreciated.

Hazard - Stormwater cover in Bike lane - Cross Rd

To Unley Council
There is a sunken cover (50mm below road surface) in the bike lane on Cross road (heading West, 50mtrs East of Goodwood Rd).
I realise the road is the responsibility of the Dept of Transport, however the service cover is clearly embossed with the word "STORMWATER" (see photos and map attached).
This cover will cause a cyclist injury or fall if hit at speed. A cyclist avoiding the impact is at risk of being struck by a passing vehicle that would not expect the cyclist to change their line of travel.
If this is not your responsibility, PLEASE PASS on to the appropriate authority.
Please also advise me on who is responsible for this type of cover. This will prevent future emails to you if not appropriate.

Hazard - Service cover on corner

 
Hello
 
Please remove the hazard caused by the service cover on the corner of Cudmore and Partridge street, Glenelg. There are two locations on opposite sides of the roundabout. The attached photo only shows one of the two hazards.
 
These hazards were reported to Holdfast council 2 years ago but no action has been taken to remove the hazard.
 
These service covers are a hazard to cyclists. Impact may cause injury or a fall. Avoiding the hazard may cause collision with passing vehicles not expecting the bicycle to change direction.
 
Please also note that this is a bikeway AND is located next to a secondary school.

Hazard - Sunken cover - 48 Birdwood Tce

Hazard - Repeated Request - Murray Tce Warradale - United Water Service Covers

There are 2 covers:

- 36 Murray Tce
The cover is sunken more than 20mm and is approximately 1 metre from the kerb.

38 Murray Tce (photo attached)
The cover is sunken more than 20mm. It is approx 3 metres from the kerb, however in the line of a cyclist when passing a parked car (see video in above link).

In addition to the risk of injury or fall from impact, a cyclist that changes the line of travel to avoid the hazard is at risk of impact from a passing vehicle.

Service Cover - 195 Anzac Hwy - Fourth request


The SIPs West of Gray St were first reported Nov 2008

One has been corrected.

Attached are photos of a second SIP in this area (195 Anzac Hwy).

One of the photos was taken 15 Nov 2008.

Anazac Hwy - Another SIP (Cnr South Rd)

The service cover (SIP) is located 10 metres West of South Rd, heading West on Anzac Hwy.

Striking the sunken cover causes a cyclist injury and damage to the bicycle.

Avoiding the sunken service cover will cause a collision with a passing vehicle not expecting the cyclist to change the line of travel.

Delays to remove hazard of service covers

are slow to remove the hazard, often not removing the hazard at all.

King George SIP - Reported to Maria Kourtesis JP, May 2009

This Service Inspection Point (SIP) cover is sunken in excess of 30mm (the ruler is green for 20mm, then red).














The sunken Service Inspection Point (SIP) will cause damage and personal injury. A cyclist changing direction to avoid the SIP is at risk of colliding with a passing vehicle. Drivers are also aggravated by having to drive slowly behind a cyclist who is in the way.

Being on a corner, this SIP is a hazard for a cyclist attempting to indicate a turn (one handed).

Reported to Maria Kourtesis JP (Liberal Candidate for Bright) at a street corner meeting at the corner of Harrow and King George streets.

The challenge was set to have the hazard removed. This was in the context of the issue that these hazards are NOT attended to, even if reported. See the topic "United Water" (click to view).

Murray Tce - Service Inspection Points (SIPs)



The video shows the impact that a sunken service cover has to a cyclist. This bicycle is a mountain bike with front suspension. The impact to a road or tourer bike is more severe.

In addition to the risks of injury or property damage on impact, there is a risk of collision from passing vehicles not expecting a cyclist to change position on the road to avoid impact with an SIP.

Letter to United Water:


29 Sept 2008

The Service Inspection Points (SIPs) shown in this video (38 Murray Tce, Oaklands Pk) have not been corrected. There were two down the road that have been raised, but no action to fix these.

United water Service have been contacted today (8381 0300) and the sip at 38 Murray Tce reported (Job Number W1162287).
