Hazard - Jetty Rd cover (5th time damaged in 2008)

 
Please find attached previous reports and a photo of the modified stormwater drain cover that is now damaged.

Please find attached a photo of the same cover taken tonight. It has been damaged within 24 hours of being repaired.
 
Also attached are the other photos from previous reports.
 
Here is a link to the reports in March, September and November 2008
 
Please advise the action the council will take to find a solution to this hazard.
